Serious question. What is the advantage of a robotic cholecystectomy over a laparoscopic one?

Lap chole is likewise minimally invasive, I suspect less expensive and isn't there an increased risk of common bile duct injury using the robot? My buds could do an uncomplicated lap chole in 15-20 minutes, so I'm honestly curious, other than the surgeon operating in 3D as opposed to 2D, what's the push for the robot here?

Either way, both are WAY better than the old open choles.

For me it’s precision, stability, and augmented reality with the robot. It has near-infrared light mode that makes indocyanine green light up the biliary tree like a road map. The dexterity of the instruments means I can hit the exact angles I want while dissecting. For really big people, most of my patients, having the robotic fourth arm from way left lateral side functions as my static retraction on a heavy liver, but also holds the falciform up and the quadrate lobe out of the way.

The robot also makes managing difficult gallbladders more straightforward for me too, gives me the option to reconstitute a fenestrated GB for example. Where other surgeons might open, I can stay MIS even on the hardest ones. You can never prove superiority, when you’re dealing w complication rates of 1/1000 like CBD injuries. The time argument is sort of a fallacy, it takes me about 3 minutes to dock and console time for easy ones is sub-20 min.

I’m also a control freak, and for me having four arms with the robot means I don’t have to rely on an assistant at all, meaning fewer variables I have to account for. When the expectation is perfection, everything perfect every time, I feel like the robot is the best tool in my hands.

Good answer and makes perfect sense.

I guess virtually no one is still doing transgastric or transvaginal cholecytectomies? Now that always seemed to fall into the category of just because you can do something doesn't mean you should.What's a few small visible incisions vs screwing with the stomach or the hoohah?
