The deadline to enter the portal in this window is Jan. 2, but players don't have to decide on their transfer destination nor do schools have to make all their additions by then.

Because the window is for the players entering into the transfer portal. It's not a window for teams adding players. There's a certain academic timeline to adding players but it's not strictly by the portal timelines.

IMO, there should only be one portal window and that is in the month of May after spring practices are over. That's it and no more.
It gives teams time to go after players and set their rosters for the start of fall practice. Besides reducing the chaos, their are other year end advantages too.
Also, early signing day for recruits should be the 2nd Wednesday in August. Things are going to get messy now that 12 teams are going to be in the playoffs. Focus needs to be placed on game preparation not chasing all over the country to check on commits.

I just read on NCAA website that first transfer window is open for 30 days and in late April there’s a 15 day transfer window. Does this mean someone can enter in December and doesn’t have to decide until the end of the transfer window in April?

They have the chance to declare again after Spring Practice. There are currently 2 transfer windows. Gives teams opportunities to fill holes but also players can bail just like our kicker last spring after the Orange/White game. It sux if you ask me!
