It is 100% subjective, and video review of it was even worse.
There is no standard or consistency. For example, in Oregon V Turd holes you could shoot a receiver in the face, so long as you turn your back on him. In other games simply flicking the receivers coat tails with your pinky was grounds for 15 yards.
i don't know how to fix it, but it is the worst part of the game.
Refs know it - they don't have the bawls to fix it.

DBs (and receivers, to some extent) have learned that it's well worth it to hold and intefere routinely, as they know that most of the time it won't be called. So now you have a rule that's there for a reason, that is ignored at worst, enforced with wild, random inconsistency at best, so the reason for the rule gets the finger from the people who make the rules in the first place.
They should grow a set and either take the rule out of the book, or enforce it as written.
